Aims
Inequalities exacerbate poor health outcomes in diabetes. Structured education in type 2 diabetes is the cornerstone of supported self-management, but many people with newly diagnosed type 2 diabetes in the UK are not offered structured education and non-attendance in those accepting an offer of structured education is high. The aim of this study was to examine the relationship between type 2 structured education non-attendance and indices of deprivation.

Methods
We wrote offering structured type 2 education to 10,322 of 11,561 (89%) people with type 2 diabetes in our predominantly deprived northern borough (contact details were unavailable for the remaining 11%). In 1,115 people who accepted structured education, we examined multiple indices of deprivation and related deciles of deprivation to type 2 structured education non-attendance.

Results
Of 1,115 patients, median (range) age 60 (21–97), 51% male, who accepted an offer of structured type 2 education, 355 (32%) subsequently failed to attend. Using the government’s preferred global measure: index of multiple deprivation (IMD), the did not attend (DNA) rate in the most deprived decile IMD-1 was 41% (114/275); in decile-2 it was 35% (70/199); in decile-3 it was 32% (40/126); in decile-4 it was 33% (37/113)… and in the least deprived deciles IMD-9 was 7% (7/69) and IMD-10, non-attendance was 0% (0/6), chi-squared test, p < 0.001.

Summary
Structured education in type 2 diabetes is pivotal to supported self-management, but programme non-attendance rises progressively with increasing deprivation, potentially exacerbating health inequalities in people with diabetes.
