A multicentre observational cohort study of cardiovascular risk and outcomes following stroke: The Liverpool-Heart And bRain Project (L-HARP)

Introduction
Improving risk prediction models for adverse outcomes following stroke, such as atrial fibrillation (AF), recurrent stroke, and cognitive impairment and dementia, would help target secondary prevention and other intervention strategies. The primary aim of the Liverpool-Heart And bRain Project (L-HARP) is to establish a prospective cohort study of adults following ischaemic stroke or transient ischaemic attack (TIA) to externally validate, refine and expand current risk prediction models for cardiovascular and cardiovascular-related outcomes.

Method(s)
Recruitment is ongoing for a multicentre, prospective, observational cohort study of 1,222 adult patients admitted for ischaemic stroke or TIA. Routinely collected data are being captured including major adverse events, supplemented by completion of simple, validated, participant questionnaires related to cognition, quality of life, and mental health. Follow-up assessments will be undertaken at 12-months, in addition to data linkage to hospital episode statistics and mortality data.

Result(s)
As of 14/06/2022, 716 participants had been recruited from 31 participating hospitals in England and Wales; n=639 had completed baseline data collection (mean age 70.2 (standard deviation (SD) 12.9) years, 58.7% (n=375) male). The mean (SD) National Institutes of Health Stroke Scale score was 5.4 (5.2), and the mean (SD) Montreal Cognitive Assessment score was 22.5 (5.4). Known AF prior to stroke was evident in 13.5% (n=86) of participants, and 20.0%, (n=128) had a previous stroke before the baseline admission.

Conclusion(s)
The data collected in this multicentre prospective cohort study will be used to answer research questions about improving risk prediction of important outcomes for individuals following ischaemic stroke and TIA.
