Buccal graft urethroplasty for bulbar strictures: the Whiston Hospital experience

Introduction
Recurrent bulbar urethral strictures greater than 2 cm are managed by graft urethroplasty. The purpose of this study was to analyse the demographics and outcomes of buccal graft urethroplasty in a tertiary referral centre. The recurrence rate was also recorded.

Methods
All Buccal graft urethroplasties undertaken by a single surgeon were included. Patient demographics, stricture aetiology and graft size were recorded. Prior endoscopic interventions, baseline symptoms at referral and at 4 months post operatively were recorded. The reintervention rate at 2 years was also noted.


Results A total of 25 BUMG urethroplasties were performed with a mean age of 52 years (range 26 – 81). The mean graft size was 4.2 cm (range 3.5cm – 7cm). Ten patients were external referrals and 9 had suprapubic catheters in place at the time of operating. Post TURP stricture accounted for 5 cases (20%). The mean number of urethrotomies was 2 (0-5). The mean pre operative IPSS and Qaly scores were 19 & 5 reducing to 7 & 1 respectively at 4 months post operatively. Mean IIEF-5 score was 17 pre operatively and 20 post operatively. The mean Peeling score was 4 preoperatively and 2 post operatively. The total reintervention rate within two years was 12% with 3 requiring dilatations.

Conclusion
Bulbar substitution urethroplasty has been successfully established in our trust.
